BIAS INCIDENT REPORT & WITNESS DISCLOSURE FORM

Forms can then be printed ( or emailed) and submitted to the building Principal, unless the incident in involves the Principal, then submit it to the Superintendent, if it involves the Superintendent, please submit it to the School Board Chair.
